It schedules patients, holds clinical charts, submits claims and keeps the patient ledger. What it isn’t built to do is run your general ledger, consolidate a group of practices or explain to your board why last month’s production figure doesn’t match the cash in the bank.

This guide sets out what each system is for, what data should cross between them, which number finance should use for which question, and the signals that your accounting setup needs a review. It closes with questions to take into any evaluation.

Key takeaways

A practice management system owns scheduling, charting, claims and the patient ledger. An accounting system owns the general ledger (GL), payables, intercompany transactions, consolidation and the audit trail. They’re complementary, not interchangeable.

Agree which summaries your PMS supplies to the ledger, how they get there and who reconciles them. Payroll and purchasing systems can feed the ledger directly too.

Production, collections and recognized revenue answer different questions. Decide which report supports each ledger entry, and who explains the differences.

Keep patient-level information out of finance wherever summaries meet the need, and confirm the safeguards that apply to anything that does cross.

Manual consolidation, slow location reporting and a controller who spends the day rekeying data are signs your accounting setup needs a review.

What we’ll cover

What each system is built for

What data should cross the boundary

Which number should finance use?

Signals that your accounting setup needs a review

Protected health information and the finance system

Questions to take into an evaluation

Frequently asked questions

What each system is built for

A practice management system is built for the clinical and patient billing workflow: appointment scheduling, treatment planning, charting, insurance claims and the patient accounts receivable ledger. It’s optimized for the chair side of the business, and it does that job well.

An accounting system solves a different set of problems. It maintains the general ledger and supports reporting under generally accepted accounting principles (GAAP), manages accounts payable (AP), records intercompany transactions between legal entities, produces consolidated financial statements and keeps the audit trail that auditors and lenders review.

The two are designed to work together. The PMS produces the operational and patient billing data; the accounting system receives the summaries it needs, books them and turns them into the management and statutory reporting the group runs on. As a group adds locations and entities, the question becomes whether that handoff, and the accounting setup behind it, still holds up.

What data should cross the boundary

The connection between a PMS and an accounting system is a defined handoff, not a full copy of one system in the other. Agree the design with your implementation partner before go-live, because the right answer depends on your PMS, your entity structure and your reporting needs.

Data that typically flows from the PMS to the ledger:

Revenue summaries by location, provider and, where your reporting needs it, payer and service line

Adjustments and write-offs at summary level

Collections and deposit summaries, so the ledger can be reconciled to the bank

Records typically maintained in the accounting system:

The general ledger and chart of accounts

Accounts payable, ideally with the invoice attached to the transaction

Payroll journals, received from your payroll system where that integration is set up

Intercompany payables and receivables between practices and any related entities

Fixed assets and the consolidated financial statements

Other systems can feed the ledger directly. At Village Family Dental, the Workforce Go payroll solution transfers pay summaries, broken down by department and provider, into Sage Intacct, and Hybrent handles purchasing. Ask your implementation partner to demonstrate each proposed flow, including how exceptions and failed imports are handled and who reconciles them.

Which number should finance use?

Most arguments between operations and finance in a dental group come down to three numbers that sound alike and aren’t.

Production is the value of the work done in the chair, usually at the practice’s standard fees. It’s an operational measure, and it lives in the PMS.

Collections are the cash actually received from patients and insurers. They should reconcile to deposits and to the bank.

Recognized revenue is what finance reports in the ledger under the group’s accounting policies, after adjustments and write-offs.

Each answers a different question, so agree up front which PMS report supports each ledger entry, which accounting policies govern revenue, and who explains the differences between the PMS, the bank and the ledger. Then set a reconciliation rhythm, for example collections to deposits daily or weekly and PMS summaries to the ledger at each month-end, with a named owner for each.

Once that’s agreed, finance can stop pulling management reports straight from the PMS and use ledger-based reports instead, with the PMS as the operational detail behind them. For how dimensions turn those ledger entries into profitability by location and provider, see our guide to dental group profitability reporting.

Signals that your accounting setup needs a review

The pressure builds gradually, as a set of friction points that become harder to work around:

Consolidation lives in spreadsheets. Finance staff export each entity’s reports and combine them by hand, and a change to one transaction means refreshing every sheet.

Location reporting takes days. Leadership can’t quickly see which offices, providers or service lines are profitable, so decisions about where to invest are made on stale numbers.

AP documents sit apart from transactions. Invoices live in a separate folder, so finding one during an audit becomes a manual search.

Manual data entry crowds out analysis. At Village Family Dental, the controller’s workload once included extensive manual journal entries and duplicate entry from payroll and patient management systems.

Audit evidence is hard to assemble. Check whether finance can trace postings, approvals and changes without rebuilding the record from separate files.

These are growth problems as much as technology problems. A setup that worked well for two or three locations can leave a lot of manual work once the group reaches ten or more. For the close itself, see how dental groups are replacing the consolidation spreadsheet at month-end.

Protected health information and the finance system

Keep patient-level protected health information (PHI) out of finance wherever summarized data meets your reporting needs. Then confirm whether anything you do transfer contains PHI, and which safeguards and agreements apply.

For Sage Intacct, that conversation covers Advanced Audit Trail, data storage rules, access controls and whether a business associate agreement (BAA) is available for your use under the Health Insurance Portability and Accountability Act (HIPAA).

Questions to take into an evaluation

When you evaluate dental accounting software for a multi-location group, ask each vendor to demonstrate the answers with your own data:

Consolidation

Can the system consolidate your legal entities in a single run, without exporting to spreadsheets?

How are intercompany eliminations handled?

Reporting

Can transactions carry location, provider, payer and service-line dimensions, and can you produce a profit and loss statement (P&L) by each without a custom report build?

What statistical data and allocation methods do per-chair or per-case figures need?

The PMS connection

What’s the integration path from your PMS: a summarized journal entry or a line-level feed?

How are cutoffs, corrections and failed imports handled, and who owns the reconciliation?

Payables and audit

Can invoices be stored with the AP transaction, so the approver sees the invoice and the GL coding together?

What does the audit trail record for each change, and how do auditors access it?

Growth

What does adding a new practice or entity involve?

How is finance staffed at peer groups of your size using the system?

Perfect Dental Management, which operates 17 offices across the US, cut its monthly close from 15 days to five and reduced multi-entity consolidations from three days to four hours.
